Condensed Abstract: Case reports and referral-based studies suggest spontaneous coronary artery dissection (SCAD) is associated with autoimmune diseases. We aimed to determine the association of SCAD with autoimmune diseases in a population-based, case-control study. We identified SCAD cases from diagnosis codes and verified the diagnosis by review of the coronary angiograms. Autoimmune disease occurred in 13 (11%) of SCAD cases and 40 (12%) of controls (p=0.93). Even after adjustment, autoimmune diseases were not associated with SCAD (OR 0.81, 95% confidence interval [CI] 0.40–1.66). These findings suggest SCAD pathogenesis is non-inflammatory and screening for autoimmune diseases based on SCAD alone is likely not warranted.
