Graphical Abstract Engineered extracellular vesicles (EVs) derived from adult dermal fibroblasts show enhanced intrapulmonary targeting and retention, and can effectively deliver therapeutic cargo to the inflamed lung to reduce inflammation, mediate tissue repair, and drive metabolic changes that favor further activation of anti-inflammatory pathways. These EVs could potentially represent a powerful tool for the treatment of acute lung injury.
