Highlights • Improved diagnosis of liver disease increases demand for fibrosis assessment. • ELF testing can be combined with indirect fibrosis scores. • The addition of ELF testing reduced referral rates by 34% in a multi-aetiology pathway. • ELF scores <9.8 have a high negative predictive value (>95%) for hepatic events. • ELF scores ≥13 are associated with rapid decompensation and warrant urgent referral.
